Help with grant proposal for exercise equipment?

I am writing a grant proposal to purchase exercise equipment for a local residential facility and the challenge I am facing is a section of the grant asking how we plan to evaluate the grant. How do you evaluate whether we got what we needed or not? The grant only focuses on getting the equipment. Any suggestions?
The measurement and analysis of your outcomes would be tied directly to the reason(s) outlined in your request for the grant.
for example…
We need exercise equipment because it will allow members of our residential facility the opportunity to exercse and improve their health …
We will evaluate the success by tracking how many members use the exercise equipment, whether the use increasese or decreases over time, and compare the number of doctor’s visits among the residents that do or do not use the equipment to determine the impact of the equipment on health of residents
